Every three years, nonprofit healthcare facilities are required by the federal government to conduct Community Health Needs Assessments (CHNA), including surveying their communities.
For the first time, the five Trinity Health hospitals in Southeast Michigan — Trinity Health Ann Arbor, Chelsea Hospital, Trinity Health Livingston, Trinity Health Livonia, and Trinity Health Oakland — are partnering to conduct a 2026 CHNA survey. The results will be used to assess, prioritize, and address the needs of their respective communities.
Anyone who lives in Livingston, Oakland, Washtenaw, or Wayne Counties is encouraged to complete the anonymous survey, which will remain open through July 31, 2026. Participants may optionally submit their name and contact information at the end of the survey for a chance to win one of two $100 gift cards; contact information will not be linked to survey answers.
“The needs identified by the community through the CHNA process guides our local strategic initiatives every three years,” said Dr. Chiquita Berg, Vice President of Community Health and Well-Being for Trinity Health Michigan. “Our Southeast Michigan hospitals are combining efforts to conduct a single survey and working collaboratively so each hospital gains a deeper understanding of the communities we serve.”
